The Shard of Agony can be obtained as a reward from one of the cursed children of the House of Skulltula in Kakariko Village after collecting 20 Golden Skull Tokens. When close to a Secret Grotto, it releases a pinging sound accompanied by a flashing icon in the top-left corner of the screen.
