Ordon Province: 1 (Ordon Spring)
Peak Province: 1 (Snowpeak Top)
Desert Province: 2 (Gerudo Mesa. Mirror Chamber)
Faron Province: 3 (Faron Woods, North. Faron Woods, South. Sacred Grove)
Lanayru Province: 4 (Lake Hylia. Castle Town. Zora's Domain. Upper Zora River)
Eldin Province: 4 (Kakariko Gorge. Kakariko Village. Death Mountain. Bridge of Eldin)

Portal History

Red Portals are created by Zant's evil magic and send Shadow Beasts to fight Link.

Once the Shadow Beasts are defeated, the portal then turns blue. These portals can be used by Midna to Warp you to any other blue portal location across Hyrule quickly and easily.

Zant can also create portals to summon various Twilight Monsters, and it is implied that Ganondorf may use them to teleport.
