The Stepadder is the quest item claimed from the Fourth Labyrinth in the first quest. (Not until 6th in the second quest).  It is used to traverse small (one-square) spans of water, pits, and even hot lava, which would otherwise block Link's progress.

Found only in Legend of Zelda, the Stepladder is an environment-active item, meaning it is used automatically when Link's environment allows for its use. There is no need to "equip" or "use" the item.
