No need to break the phases into sections. Fixing many typos and rewriting things for clarity. She is definitely not the first or only boss damaged with a sword.
(Helping out the page, and also adding in a Trivia Segment.)

Blizzeta has two main forms during the battle.
Blizzeta has two main forms during the battle. She will first take the form of a large iceberg and will slowly follow Link around the icy chamber in attempt to deal contact damage. The Ball and Chain must be used to chip away at the layers of ice and eventually damage her. After each hit, Blizzeta slides around the chamber at greater speeds, occasionally summoning a ring of Mini Freezards around her and launching them at Link. After enough hits, Blizzeta is briefly freed from the ice.

Phase One
Blizzeta will then rise into the air and let out a scream, encasing herself within another ice barrier surrounded by huge ice spikes. The camera switches to an overhead view, where reflections of Blizzeta and the ice spikes can be seen in the icy floor as they hover over Link and attempt to crash down on him. After the ice spikes fall, Link can break them with the Ball and Chain, sometimes releasing recovery Hearts. They can also form a circle around Link in order to trap him for Blizzeta's attack. When she falls, Link can attack her with the Ball and Chain as well.
During her first form, Blizzeta will first take the form of a large Ice-Burg and will slowly fallow Link in attempt to deal contact damage. The Ball and Chain must be used to chip away at the layers, after being hit, Blizzeta will spins around the chamber, and after a few more hits, she will summon small ice around his as she spins and will lunch them at Link, she must be hit until barely anything is left.

Phase Two
By attacking Blizzeta three times with the Ball and Chain, she will be defeated. After the fight, Yeta's husband, Yeto, appears and embraces Yeta as she recovers, releasing a flurry of Hearts and a Heart Container around them.
Afterwards, Blizzeta will encases herself within another ice barrier and rises into the air, summoning huge ice spikes around her, which hover over Link in an attempt to impale him. After they fall, Link can break them with the Ball and Chain, releasing recovery Hearts at times. They can also form a circle around Link in order to trap him for Blizzeta's attack. When she falls, Link can attack her with the Ball and Chain as well. Link can use the reflection on the ice floor to see where Blizzeta and her ice spikes will fall.

Trivia
*Blizzeta is the second of three Bosses in TP to be a friendly character possessed by a twilit object rather than an enemy, who then returns to their original form after being defeated. She follows Fyrus and precedes Possessed Zelda in this trend.
Trivia
* Blizzeta is the first and only boss in The Legend of Zelda: Twilight Princess that Link can damage with his sword.
** Additionally, Blizzeta is the second boss in The Legend of Zelda: Twilight Princess that can not be finished off with a Finishing Move, the first being Stallord.
* Blizzeta is the second boss in The Legend of Zelda: Twilight Princess to be a character posses by a shadow-like object, rather then be an enemy, the first being Fyrus
** Also, Blizzeta is the second boss in Twilight Princess to return to her original form after she is defeated, rather than perish like the rest, the first being Fyrus.
*** This is due to the fact that both Darbus and Yeta are victimized characters, rather than just being enemies.
